Evaluation of Skeletal Mid-Facial Asymmetry Using Analyze 9.0

We establish a plane of reference using the inferior orbital foramen and the superior surface of the external auditory meatus to align the head position.

We then view a number of transverse slices, define the malar bones as objects (See 2 in green and 3 in yellow above), and measure the volume differences between the left and right sides using the total volume for all the slices on each side.

Left side volume : 27,262 mm3

Right side volume: 13,217 mm3

The measurements above show that the left side volume is twice as great as the right side.

This patient is right side under-developed

Before

After 4 months treatment

A post treatment CBCT scan will be available for skeletal evaluation in 2 months.

“ALL LIVING SYSTEMS HAVE INHERENT SELF-CORRECTION MECHANISMS!”

Happy Halloween!

Above, we see unilateral bone development (red) as a result of Homeoblock™ treatment. This is an example of self-correction. This patient’s left side was less developed before treatment.

This patient shows right side underdeveloped evidenced by sagging lateral canthus of the right eye and thinner right side upper lip.

The green area in the above transverse section shows the pattern of bone development after Homeoblock™ appliance treatment, with an increased amount bone development on the right side.

(Analyze 9.0, the Mayo Clinic)

The above transverse slice from the CBCT shows 2mm of bone deposition and repair under the right orbit (green).

(Analyze 9.0, the Mayo Clinic; CBCT registered before and after scans)

The patient after Homeoblock™ appliance treatment shows improved mid- face volume.

Note: Sclera under the eye does not show up in the after photo.

EVIDENCED-BASED CONCLUSION:

Homeoblock ™ appliance treatment stimulates the body’s self-correction mechanisms!

How Do I Incorporate the Homeoblock™ into My Practice?

Facial anti-aging is a 12 billion dollar a year business.

Our faces do not age symmetrically.

The less developed side of our face ages more rapidly.

The patient’s left side is less well developed.

The less developed side shows deeper lines, depressions and wrinkles.

Homeoblock™ treatment will develop the less developed side for our patient, and improve facial symmetry.

The result is a younger appearance with fewer lines, wrinkles and depressions.

There will be noticeable results after as little as three months of treatment.

For your patient demonstration, create a chimera (flipping the left side of the face over the “y” axis to create a new face) of the underdeveloped side using PowerPoint.

This exaggerates the aging and is a very powerful tool to get your patient’s attention.

Contact us to learn this simple technique performed by the PowerPoint software.

This is an ideal patient to treat for an underdeveloped maxilla.

Note: Depressed mid-face and thin upper lip.

Please note fuller upper lip and improved mid-face volume after 6 months.

Treating the underdeveloped maxilla in a seventeen year old patient with the Homeoblock™

“The maximum genetic potential for growth can only be achieved by freeing up cranial restriction”. “This can be achieved by harnessing a corrective force, the self corrective force inherent in living systems”

Gavin A James, Dennis Strokan

Dr. Belfor at Derek Mahony’s EODO DownUnder, October, 2009

(see below)

Treating the underdeveloped maxilla in a seventeen year old patient with the Homeoblock™ frees up the cranial sutures and harnesses the corrective force.

The left central and lateral incisors are forced labially due to the tight occlusion.

Below: We see before and after six months of Homeoblock™ treatment. We see the before treatment is on the left side and after treatment is on the right side throughout.

Despite the increase in the size of the alveolus the spaces between the teeth begin to close. We postulate that the teeth move into position due to temperal-spatial patterning. (Genetic programming)

We see wider more prominent cheekbones, fuller lips, more level eyes and a reduction in the bagging under the eyes in the after photo.

Changes in the maxilla results in an enhanced profile.

Morphometric analysis shows an increase of almost 2mms in the malar region, the upper lip and the pre-jowl and marionette area.

We see improved symmetry and fullness of the upper lip.

This presentation is a good example of enhancing facial appearance along with tooth movement over just six months of night time only adult treatment with the Homeoblock™ Appliance.

Treating Class lll With a Bi-lateral Crossbite With the Homeoblock™

After 12 Months

Please note arch development and leveling of the occlusal plane. The increased vertical height concurrent with the open bite enhances facial esthetics.

This patient reports better breathing and improvement of head and neck pain. Treatment will continue for 12 more months.

Note: Decreased laxity of the lower eyelid and the fuller upper lip

More Level Eyes

Increased Mid-face Volume

Class lll patients with underdeveloped maxilla can benefit from Homeoblock™ treatment. We do not expect to correct the cross bite. The facial esthetic, the functional improvement, postural and breathing improvement, makes the treatment a success.

How do we get these results?

“Epigenetic Orthodontics

Intermittent light forces as small as 1 gram applied physiologically during the act of swallowing to the periodontal ligament produce the changes in the alveolus and the tooth position.

The periodontal ligament functions quite like the boney suture. It has high vascularity and fibrous tissue that can differentiate into bone tissue. The PDL contains nerve fibers (receptors) blood vessels and mesenchymal cells that are undifferentiated, or partly differentiated, cells that act as adult stem cells, and can produce osteoblasts, osteoclasts and fibroblasts. The receptors in the PDL let us know where the position of the jaw is at all times. When we stimulate the development with the intermittent forces, the body begins to self correct. Ultralight signals (less than 1 sec and less than 1 kg) are detected by mechanoreceptors on the surface of the periodontal and periosteal cells. When the light forces are applied to the teeth, the blood vessels are compressed on the pressure side and dilated on the tension side. Oxygen tension begins to change and prostoglandins and cytokines are released. Chemical messengers are affected and enzyme levels change. There is an increase in cAMP (cyclic adenosine monophosphate) and cellular differentiation begins within the PDL.

In relation to the changes we see in the face:

Biomechanical stresses are linked to gene expression through the production of BMP-4. (Bone morphogenetic protein)

Due to signal transduction, a cascade of events is initiated resulting in gene transcription and mRNA biosynthesis and tissue formation.

An Exit Strategy for Sleep Apnea

The biodynamic flux produced by the Homeoblock™ appliance provides an adaptive environment in which the functional and growth potential of all cranial structures can be optimized. This includes airway form and function and the positional movement of the teeth to a more integrative functional location within the context of the bony matrix and optimized neuromuscular feedback.

Before and after cone-beam volumetric tomography airway analysis using Analyze 9.0 developed by the Mayo Clinic.

See Below:

After registering the before and after cone-beam volumetric tomography data using the basion region as a reference point, we can see the changes that have taken place in the pharyngeal airway, the maxilla and mandible, and the inferior turbinate. The green area shows the pre-treatment airway and tooth position. (Please note the lower central incisor as well as the airway.). We see that the maxilla and the mandible have come forward along with the tongue. Furthermore, we see that the posterior pharyngeal wall has toned up to widen the airway. We have a 360 degree improvement of the airway space. The lower turbinate has increased in size.

This patient reduced his apnea-hypopnea index (AHI) from 28 to 1, and no longer requires a CPAP.

Note: Positional movement of teeth to a more integrative functional position.

Bio-Research’s T-Scan results after one year of patient treatment with the Homeoblock™ appliance contributed by Dr. Arnold Valdez, illustrate more functional teeth positioning.

Sleep Apnea; A Possible Exit Strategy without Surgery

What is Epigenetics? This new science reveals that information that controls biology starts with environmental signals that in turn, control the binding regulatory proteins to the DNA. Regulatory proteins direct the activity of the genes.

As we know, sleep apnea is currently treated with a CPAP. If there is moderate to severe sleep apnea, the patient may benefit from a machine that delivers air pressure through a mask placed over the nose while sleeping.

Another option is wearing an oral appliance designed to keep the throat open. A CPAP is more effective than an oral appliance; however, an oral appliance may be easier for the patient to wear. Some appliances are designed to open the throat by bringing the jaw forward, which can sometimes relieve snoring as well as mild to moderate obstructive sleep apnea. Both treatments are without an exit strategy, and are for life.

If the airway is extremely limited by size or volume, a CPAP or surgery are the only alternatives. Moving the jaw forward can maintain the airway, however, if the size of the airway is severely compromised, maintaining it will not help apnea.

The Homeoblock™ has been shown to increase airway size or volume.

Before Treatment:

After treatment

After treatment, this patient reduced his apnea from a Respiratory Disturbance Index of 45 to 5.

Vultus software with a cone beam CT allows us to look at the pharyngeal airway before and after treatment. Please note the change of width in the axial dimension.

This patient no longer wears a CPAP or an oral appliance.

We suggest that these changes are due to the expression of redundant genes or DNA alleles for craniofacial morphogenesis. The gene expression was stimulated by wearing the Homeoblock™ appliance at night. An environmental signal essentially results in the formation of regulatory protein which switches on the gene DNA, which in turn results in MRNA biosynthesis, a messenger protein and morphogenesis.
